RATES

My rates are competitive and for a limited number of clients, I can offer a sliding fee scale. If I cannot provide the services you would like at a fee that you can afford, I would be happy to refer you to colleagues who may be able to work with you.

INSURANCE

I do not accept insurance at this time. If you have out-of-network benefits, your insurance may reimburse you for psychotherapy. Each insurance plan is different and I encourage you to review your plan's coverage, but many will reimburse at rates of 50-80% once a deductible has been reached. I will provide you with an invoice so that insurance companies can process your claim.

CONFIDENTIALITY

Your privacy and right to confidentiality are protected by the law and the American Psychological Association's Code of Ethics. Information about you or your treatment cannot be disclosed without your consent, except in a few rare cases. I am required by law to inform legal authorities and/or potential victims 1) if you make a serious threat to harm yourself or another person, 2) if I believe that a child, older adult, or other dependent person has been/will be abused or neglected, 3) if I am ordered by a court to release information as part of a legal proceeding.

NOTICE OF PRIVACY PRACTICES AND OFFICE POLICIES

I am required by law to provide you with a copy of the HIPAA Notice of Privacy Practices so you can understand your rights and protections related to the use and disclosure of your identifiable health care information. A Notice of Privacy Practices and a Treatment Agreement Form, which explains my office procedures and includes important information about your confidentiality, will be provided to you at our initial appointment.
